Intake form

You will receive by email a client intake form and other paperwork.

The Client Intake Form is where you record any medical history, past/present injuries and other physical conditions I should be made aware of. When we start our first session, we will review the Client Intake Form together. You can share with me the reason(s) you are coming for a session.
